Do I have to be referred by my family doctor to come to your office?
We do not require a referral from your family doctor. However, your insurance company may require you to obtain an insurance referral from your family physician for your visit.
How can I get a copy of my medical records?
You can call the office between 8:00 am and 4:00 pm Monday thru Friday and ask to speak to someone in medical records. You can also access your records through our Patient Portal (if you are not already registered for this, please inquire about it when you call the office).
Where are you located? What color entrance are you?
We are located at the Robinwood Professional Center, in the GREEN entrance, Suite 126 on the first floor.
Do I need to bring anything with me?
Please bring your insurance card, a photo ID and your copay or co-insurance. You will also need to bring a list of your current medications and dosages, drug allergies and the surgeries you have had in the past.
Do your doctors see children?
Yes, our doctors see patients of all ages.
What happens if I have no insurance?
Our self-pay policy requires a $100 deposit at the time of the visit. Any amount due over that can be set up on a payment plan with our billing department. If you prefer to pay in full at the time of service, we will discount any procedures performed that day by 20%.
How long will my appointment take?
New patients can expect to be at our office for approximately one hour. If your problem is ear-related, you should plan on being at our office for approximately 1.5 hours.
Will you send my prescription directly to the pharmacy?
Yes, we will electronically send, fax or call in your prescription to the pharmacy. Please make sure we have your current pharmacy listed in your chart to avoid delays in the transmission of your prescription.
Will you send my current office note to my doctor?
Yes, we send your office note to your primary-care physician and/or referring doctor within 24 hours of your visit. Please make sure we have your current primary-care physician and referring doctor noted in your chart.
Will my insurance cover today’s office visit and associated procedures?
We have contracts with many insurance companies, but to be sure you have coverage and to review your benefits, please contact your insurance carrier prior to your visit with us.
